만성질환의 건강문제는 관리될 뿐 완치되지 못한 채 대상자와 가족에게 다방면의 문제를 일평생 지니게 하며, 만성질환자들은 의료기계 기술에 의존하여 생명을 연장하는 경우가 많아지게 되었다(Jun, & Jung, 2000). 투석환자들은 생명 유지를 위하여 지속적으로 신대체 요법을 받아야하고 치료의 중단은 곧 생명과 관련이 있기 때문에 환자들은 개인적인 생활에 제한을 받고 있다. 또한 투석기술의 발달은 투석에 의한 부작용과 불편을 많이 해소시키기는 하였으나, 환자들은 신장 이식술을 받지 않는 이상 평생 동안 투석치료를 받아야 한다는 사실과 질병 및 치료에 따른 합병증에 대한 우려 등으로 인하여, 다른 만성질환자들에 비하여 유의하게
압박감이 많은 것으로 나타났다(Kim, Min, & Han, 1994). 우리나라 신 대체 요법을 받고 있는 환자 수는 혈액투석이 25,335명, 복막투석이 7,569명, 신장이식이 8,987명으로 전체 환자수가 41,891명이었다(The Korean Society of Nephrology, 2004). 만성신부전 환자는 평생 동안 질환과 투석에 대한 조절을 통해서 정상적인 생활이 가능한 질환이므로 환자 자신이 질환에 대해 이해하고 치료에 대한 지식을 가지고 이를 이행하는 것이 가장 중요하다(Linda, 1982). 이행은 병원방문, 약속일을 지키는 일, 치료를 꾸준히 계속하는 일, 식이요법, 약물복용 등 처방을 실천하는 일, 개인행동 처방을 실천하는 일 그리고 개인행동을 억제 혹은 변경하는 등의 여러행동이라 정의하였다(Kasl, 1975). 만성질환에서의 이행은 대상자와 제공자 모두에게 중요한 고려사항이 되고 있는데 이
는 대상자가 자신의 일상적 건강관리를 실천하려는 의지에 따라 질병치료의 성패가 달려 있기 때문이다(Kim, et al.,2001, p. 397).

Purpose: The purpose of this study was to examine the relationship between the group of Self-efficacy and the group of compliance of patient in hemodiafiltration.
methods:The study involved 100 patients undergoing hemodiafiltration between April and May, 2005 from 5 hospital in G province. The questionnaires used for measuring Self-efficacy, self-reported compliance of patient. Physiological indicators include pre-dialysis serum potassium level, pre-dialysis serum phosphorus level, inter-dialytic weight gain. The data were analyzed with SPSS 12.0 program with percentage and canonical correlation.
Results: In the relationship between the self-efficacy domain and the compliance of patient domain, the main influencing factors were efficacy expectations(-.713), self-reported compliance of patient(-.870).
Conclusion: These results suggest that nurses should provide the cognitive behavioral support, especially focusing in increasing the level of self efficacy.
